Referrals

We welcome referrals from families, social workers, healthcare professionals, and local authorities. Our referral process is designed to be clear, confidential, and efficient. To refer an individual to our service:

Gather Client Information

Prepare details about the person needing care, including their full name, age, support needs, and any diagnosis (learning disability, mental health condition, etc.)

Complete the Referral Form

Use the online referral form on this page or download our referral pack. The form will collect basic information and a summary of support needs. (All information is sent securely to our admissions team via email.)

Include Supporting Documents

Attach relevant assessments or reports, such as care needs assessments, health reports, or a current care plan. These help us understand the person’s requirements from the outset.

Submit and Wait for Response

Once submitted, our admissions coordinator will review the referral promptly. We usually respond within a few working days to discuss suitability and next steps. We may arrange a visit or a phone meeting with the individual and family.
